T-4 Forms

USANA will not issue a T-4 Form to Canadian Associates regardless of how much they have earned throughout the year, however, are still responsible for reporting their income to Revenue Canada. With this, an Earnings Reconciliation Statement may be located online, usually, around mid-January, on The Hub:

  • Log onto The Hub
  • Click on the “My Account” icon,
  • Click on “Tax Information”
  • Select “Annual Tax Statements”

Availability Date

Reconciliation Statements are usually available online around mid-January the following tax year. The Hub currently contains tax Information from 2013 forward.

2022 Reconciliation Statements will be made available on January 24, 2023.

*Reconciliation requests will not be accepted unless there is documented technical error, a transfer that transpired during the tax year, or the account was closed during the tax year. In such events, requests can be requested through Customer Service.

*Please note that if a Canadian Associate does not provide USANA with a Tax ID or Social Insurance Number (SIN), USANA is unable to put any type of reconciliation information for the account until there is one on file.
