Switching Main Applicant and Co-Applicant on a Distributorship

To switch the main Applicant and Co-Applicant on a Distributorship, fax or mail a new, properly completed Associate Application and Agreement form. The individual wishing to be the main applicant should complete and sign the form as the main applicant. The individual wishing to be the co-applicant should complete and sign the form as the co-applicant. Please write the current ID on the top of the application to avoid double registration. Include a cover sheet explaining your request.
